Why Did Our Audio Loop During the Live Stream?

We started the live stream and everything seemed to be going great, and then the audio from the first 60 seconds started to replay and loop back into the live audio stream. There were constantly 2 or more audio streams going through the whole event. No one could understand anything that was said.

Does anyone know why this could have happened?
